Trinity Home Healthcare’s Supported Living program, in alignment with the Ohio Department of Developmental Disabilities (DODD) guidelines, empowers individuals with developmental disabilities to thrive in their own homes. Our state-compliant, compassionate, and dedicated Direct Support Persons (DSPs) provide personalized direct support services, leveraging their extensive experience and expertise to foster independence and self-directed living. Our DSPs possess exceptional communication skills, both written and verbal, and a deep understanding of developmental disabilities. They embrace a person-centered approach, recognizing and respecting each individual’s autonomy and right to direct their own life.

Nursing
People with complex medical needs may require nursing services that are typically provided through Medicaid State Plan services. We provide Skilled Nursing Services, providing comprehensive medical care and support to individuals in the comfort of their own homes. This integrated approach ensures that our clients receive seamless, holistic care that addresses their unique needs and promotes optimal well-being.
Community Respite
This service pays for a person to receive support from other caregivers when their regular caregiver is not available or needs a break. Community Respite must be provided outside of a person’s home at recreation centers, camps, or places with organized community programs or activities. Home & Personal Care Services
Home & Personal Care Services
Homemaker/Personal Care, often called HPC, supports a person to be more independent while meeting their daily living needs. Homemaker/personal care advances the individual’s independence within his/her home and community and helps the individual meet daily living needs.
Community Transitioning
This service can support people as they transition from living in a facility to living in a house or an apartment, with others or on their own. It can pay for one-time household expenses like the security deposit for an apartment or start-up fees for utilities.
Medication Administration
Medication administration training and certification developed by DODD authorizes caregivers to perform a variety of tasks for people with many different medical conditions.
Employment Support
Employment Support can help a person learn how to do a new job or get better at their current job. It helps someone to learn about different job tasks, technology that can make their job easier, and how to work more easily with other people.
